JOB SUMMARY

Within the framework of Holy Family Community Philosophy and Goals, the RN Team Leader assumes the responsibilities related to the provision of quality medical and nursing care (body, mind and spirit) for the residents in the Holy Family Community on her/his shift.

QUALIFICATIONS

Possesses a current and valid California RN license.

Is able to demonstrate clinical nursing skills. Has a minimum of 2 years of successful professional nursing experience in geriatrics and long-term care. Demonstrates supervisory experience.
Ability to interact in a constructive manner with geriatric residents.
Ability to work as scheduled and consistently demonstrates dependability and punctuality.
Ability to work with CNAs, immediate supervisor, and personnel from other departments.
Demonstrated discretion in all matters related to the assignment, including confidential information seen and heard.
Ability to speak and understand English to follow directions to perform the essential functions of the job in a thorough and safe manner.
Is respectful of the culture of religious communities.
DUTIES & RESPONSIBILITIES
Knows and implements the Mission & Values of the Sisters of St. Joseph of Carondelet.
Knows and uses accurate information concerning the Policies and Procedures of the Facility particularly with regard to privacy issues.
Demonstrates awareness and practical problem solving ability to assure Sister/resident safety.
Has working knowledge of Fire Safety Procedures and the Evacuation Plan. Participates in a constructive way in practice drills.
Organizes, coordinates and supervises the activities of the nursing team on her/his shift.
Evaluates the health status of each sister daily.
Participates in a constructive way as a member of the nursing team.
Orients, teaches and evaluates medication/treatment nurse and the nursing assistants on his/her shift in the fulfilling of the requirements of their job description.
Dispenses medications on her/his shift alongside other licensed staff person on duty.
Provides treatments and support residents.
Provides written documentation in each sister resident’s health record for care provided on each shift, completes treatment log, oversees that monthly vitals are performed and documents accordingly.
Charts weekly progress notes on assigned sisters and documents changes of condition.
Completes follow-up on Sisters returning from medical appointments, coordinates care and interfaces with physician and nurse practitioner, reviews health chart and notes physician’s orders.
Gives both complete and accurate reports to next shift, as well as, immediate supervisor.
Perform other related duties incidental to the work described herein or as may be assigned by the Director of Health Services.
PHYSICAL DEMANDS AND WORKING CONDITIONS
Ability to transfer residents.
Ability to push occupied wheelchair and push/pull medication/treatment cart and other equipment.
Ability to lift or carry 50 to 60 lbs
Ability to stand, bend and walk throughout the day
Ability for rapid mental and muscular coordination simultaneously.
Ability to see and hear, or use prosthetic devices in which to use these senses adequately, to ensure the requirements of the position are met.
Able to grasp firmly with both hands and use fingers to manipulate small objects (i.e., pills, needles, etc.).
Ability to cope with the mental and emotional stress of the position.
Ability to perform effectively with frequent interruptions.

Pay: $39.00 - $41.00 per hour
